Sounds good. I'd add a "true ghost", something that isn't referenced by the game in any way, with little to no occurrences, but enough that the average player will encounter it at least once or twice.

I know what you mean, this chick at my church had arachnophobia and I hardly believed her but the day that I was in the same room with her and she spotted a spider, she froze like a block of ice. I didn't notice the spider until I saw her frozen unable to talk, pretty sad desu, got up and picked up the spider and gently put the little guy outside.

The only times I've been scared with a game is when I'm going through a tough part and I'm about to lose a lot of progress. Do a slow, methodical difficult action game with permadeath and a creepy theme and you are set.
