Everything I am going to start with is normal run of the mill growth or changes to myself, and this growth or changing can start immediately, as long as anything before the date this post was made remains unaltered, save for my genetic potential for these things to manifest after the date of this post.
I would like for the changes to begin within 10 days after the next shift and come to fruition 2 months after they begin.
First I would like a larger penis. 10 inches in length, 6 inches in cicumfence, with a consistent thickness for the whole length of the shaft, and an aesthetically pleasing head.
Second I would like for my acne to clear up.
Third I would like for my knees to fully heal to the point where they no longer have any reason to ache, and function perfectly. And I would like for this state to persist no matter how much I run or lift or age.
I would like a similar effect for my every joint in my body, especially my elbows, wrists, shoulders, back, hips, spine, and neck. For them to be in perfect condition, undamaged, function perfectly, and stay that way regardless of age or how I treat them.
Fourth I would like for my gynecomastia to go away.

Next I would like a few objects, that I will describe in detail to appear on the small table next to my bed within 24 hours after the next shift. They should appear on top of anything else on said table so they can fully manifest regardless of what else is already there.
First a spiral notebook with a red cover and 100 pages that I can use to alter my appearance.
My appearance includes the size, shape and color of all features of my body.
Anything written in it will take effect immediately after i write "go"
Writing "stop" will nullify anything written above or the left if the word 'stop'
If anyone else writes in the notebook nothing will happen.
I should always turn out as intended, or pictured in my head assuming it matched my writings.
If the notebook is destroyed all of it's effects will be nullified.
If anything that is written that would change my body into something that can not support life in the same state of being as I am now, no change will take place.
Second, a notebook with a blue cover that follows the same rules, but about my abilities rather than appearance.
My abilities are described as things my body is capable of, both physical or mental.
Changes of abilities should never effect my appearance.
No change should create an altered state of consciousness.

If any ability or appearance change defies the laws of physics nothing else will be effected other than me, and no effects other than the exact change written about myself will occur, and I will survive, and not be incapacitated or hurt in any way, and they will function as intended. This is especially relevant to any super powers I may write.
Both books have the rules put, how I worded them, on the first page.
Stray marks will have no effect, an all effects should turn out as intended with no side effects, unless they break one of the previously mentioned rules in which case all effects are nullified.
