It's not plaque or bad teeth. It's a sort of glaze-like black tooth lacquer traditionally worn by Japanese women way back when.

See, traditionally, in Japan, in Southeast Asia, in parts of China and the Pacific, those sort of shiny black teeth were considered very attractive, so women would basically paint their teeth glossy black. You can still find some old Chinese and ASEAN (SE-Asian) women in rural areas who paint their teeth black, but the practice has basically completely died out in Japan due to American influence. Now in Japan it's pretty much just done as part of historical reenactments and reconstructions of traditional garb.

That woman, with her shaven eyebrows, white-painted face, rose-painted eyes and lips, and black-painted teeth, would've been a real knockout in Medieval Japan. Although, now that I think of it, at least in the Heian era the thing to do was to just shave off most of the eyebrows save for the innermost (as in toward the nose) like fourth or fifth of them, which would be shaved into little circles.
