I don't know that's just how it works here. You go to a dentist that takes your insurance and they send you a bill in the mail, and I can't remember when I last went but it definitely wasn't two months ago, and I paid the bill that came in the mail soon after that appointment (it was well over $38 for a normal visit)

Because people getting $2,500 IRDs would probably prefer not to have to pay in cash immediately after having a bunch of titanium screwed into their jawbone. Plus you can forward the bill to your health plan and have them pay it.

Just go see them and ask them for a full statement of account.
You should be able to square the payments they think you've made with what your bank statements say you've actually paid, and the charges with the procedures they've actually done, as recorded in your medical records.

Here it's the way that what healthcare covers is directly managed with your healthcare provider, you do not even get a letter or anything, you just go to the dentist, give your card, let what you have the right for made, for example one normal checkup per year, take your card and go.
For everyting else like professional cleaning etc you get a bill in the mail a few days later. most dentists delegate this to specialized companies to manage the billing, the first time you actualy have to give your written OK that you allow this company to bill you for your dentist.
