I need some suggestions on what could be causing the following:

>"Windows failed to start" every time I boot
I tried the following in roughly this order:
>disconnected all drives apart from the Windows HDD
>new SATA cable plugged into a new SATA port
>tried new RAM sticks
>reinstalled Windows
>reinstalled Windows on a completely new HDD with new SATA cabled and tried plugging it into different SATA ports
Reinstalling Windows on a new drive fixed it for a very short time, but after about a week the problem comes back.
Anyone got any clue what could be causing this? I think the only options left are the mainboard.

Scratch that, I just remembered that I bought a new mainboard and replaced the old one as well, so I doubt it's that.

Looks like you tried everything, I had similar issues with a laptop that kept overheating and crashing causing a lot of disk errors. I think the thermal pads it had where old. So, I replaced them and applied new thermal paste for the CPU.
Does your PC randomly crash? Random crashes can create disk errors.

I've not had any crashes fortunately. The thermal paste I reapplied just 5 months ago, and all the temps are fine.
chkdsk and smart show no problem with the drives either.

If you're using Windows 7, that exact message means you've switched on secure boot.
Windows 7 is not signed for secure boot, so it will always fail to boot and give you that message.
If it keeps turning on by itself, you probably need an new CMOS battery.

Thanks for the tip. Next time I am forced to restart my PC I'm gonna have a look in the UEFI.
