Help I've seen an image a thousand times on 4 Chan and

Is it pixelated ?
Does it have trumpet in it ?
Is it pic related ?
Is it skeleton ?

Not pixelated
It's smoke trailing into the contours of a face with only one eye properly formed.
No skeleton

It's got a very sandman, the crow or even joker feel to it.

No trumpet

It's an illustration (at least the one i saw was).
The smoke comes from a lower left postion into teeth, swirls into and up the length of a nose, left under a mostly blank left eye and across into a right eye.

It's a little like this.
The face is more human looking and detailed of course

Giving this a hopeful bump as my google fu has failed me.
