I'm pretty sure in Shiki a girl gets her head crushed by a tractor. There's also Blood+ but I've only seen web's but it looks pretty violent.

JoJo's Bizarre Adventure isn't nonstop gore, but when there's gore it gets pretty intense.
Black Lagoon isn't very gory until the OVA, but it's really violent and bloody.
