ITT: games that were exclusively released in Japan (or other non-English-speaking countries) and either have an English patch or don't require one to play
I've seen lots of these threads about RPGs, so let's try focusing on other genres, please.
Adventure Island IV
Rendering Ranger (Super Famicom).

Yume Penguin Monogatari
Bio Miracle Bokutte Upa
Jajamaru no Densetsu
Moai-kun
Doki Doki Penguin Land MD
Nazo No Murasame Jou
Youkai Douchuuki (arcade version is in english)
None of the Pingu games need a translation.

MW4 was released in the west with an official translation though.
Crisis Force
Getsu Fūma Den. Sidescrolling action adventure with an overworld map. Think Zelda 2 except good, and mixed with TMNT1 since they reused the engine for that game.
There isn't much text but it's better to get the translation still.
Also, during the faux 3D segments, an item you can use can turn on the lights.

I do too, also forgot to shill about it more but Youkai Douchuuki is my favorite game at the moment, it's a sidescrolling action platformer about a kid escaping hell.

Looks like a really interesting puzzle game.

Even better, people actually extracted the officially localized Genesis ROM from the emulated versions on several modern consoles and put it on the internet, meaning you don't even need the patch to emulate it anymore.
Do-Re-Mi Fantasy - Milon no Dokidoki Daibouken
Game's pretty comfy

>tfw my childhood crush has this game

She eventually moved and we never talked again. But I still replay this game a lot.

I remember going "oh this game is pretty damn easy" early on and in later levels being "godammit ".
