If you're good at the game, not terribly. Most of the difficulty differences stem from the Japanese release's more varied damage outputs from enemies. In the Western release everything hits with equal brutality.

There are some other differences, though. Dracula's last form in the Western release is notably better since his lasers are a bit more dangerous. There may be some other big things I'm not remembering, but I think that's the gist. Either version is great.

International version has quite a few reworked sprites most of which look better and it has a "second quest" but no titties.

This happens a lot. The US version tends to tone down the quality of subweapons you get right before bosses, so you're punished pretty hard for dying in a boss fight. Happens with Dracula too.

Even though so many people seem to think it's shit nowadays, I've always loved Simon's Quest. It's the nostalgia goggles and I know it, I just don't care. I still love that game.

There was a romhack that added new characters as well as changed the music but it's only for the japanese version sadly. Perosnally i'd take a music only mod for the english version.

Depends. It's a little easier innately because enemies only do a set amount of damage individually, as opposed to every enemy doing the same amount of damage depending on the level.

The NTSJ becomes insanely easier if you get Grant though. His wall-climbing ability means he has the ability to skip entire areas sometimes, and the fact that his dagger travels the full length of the screen almost makes him entirely broken. He makes most of the more difficult parts of the game a complete joke, including even the final boss, due to the fact that he's always able to keep his distance from the enemy and fire away with few if any repercussions. The thing about most of the bosses is that there is usually a safe zone of some kind, and they require you to put yourself in harm's way in order to attack them. With NTSJ Grant however, he's able to nearly always keep at a safe distance and dispatch both enemies and bosses without having to deal with putting himself in harm's way. With his jumping and his projectile, he practically turns it into a Mega Man game.
