Are Genesis games supposed to played at 224p or 240p? I've been playing 240p my whole life but I was fiddling in Kega Fusion today and it looks like "NTSC" aspect (240p) is stretched and PAL is pixel perfect? I'm very confused.

The accurate way is to is to display 240p with full overscan areas as 4:3.

This. Genesis native res is 224 tall but if you're playing an NTSC game, you want 240p with letterboxing type bars at the top and bottom though most TVs will overscan the sides well off the screen and the top and bottom pretty even. This is why you will get your score partially hidden on a lot of NES games but it looks perfect on Genesis. If you're emulating, then either is fine as long as if you make it full screen you use integer scaling.
