The 32X addon expanded the Mega Drive's capabilities in 4 ways:
Increased the color depth to 32768 from 61.
2 new CPUs inside which did software 3D much faster than the Mega Drive CPU.
The video encoder outputs much cleaner composite (presumably RF too) than the Mega Drive.
2 additional audio channels for a total of 8 (16 with Sega CD).

How does it sound compared to the stock Mega Drive?
What are the best and worst sounding games that used the extra channels?
What is the signature sound of the 32X? For example, the SNES has a unique, unmistakeable trumpet sound.
Adding in the extra audio channels didn't change the Genesis' sound, besides adding in more wiggle room to have the drums be PSG samples.
If your expecting something dramatically different from the stock Genesis, then your not getting it here.
That said, Cosmic Carnage, Tempo and Knuckles Chaotix are probably the best sounding games on the system.

More PCM channels, so at that point you didn't have to worry about SFX stealing from your music.
I don't own 32x Doom btw so I wouldn't know if sounds cancel each other out. This is a pretty common thing but there are a few exceptions.
