Why would anyone be so sadistic as to design a game like this? Even assuming it was the 80s and no one knew anything about proper game design, did no one who play tested it think it was too hard and speak up? I mean it's just common sense that people aren't perfect, yet they still made games that demanded perfection.

Modern games are FAR worse, for FAR worse reasons. Classic games at least tried to be entertaining (though admittedly hard as shit for shitty reasons)

Yeah, it was the 80's the designers and programmers played and tested the games. Because they were so used to the games, they felt easy for them and usually ended tweaking up the difficult before release.
Games now had real betatesters that don't have any involvement with the developing of the game, and games are targeted to a larger community, includying casual gamers, so a really "80's difficult" game, wouldn't sell too well.

Replay value. There's a longplay on youtube of that game, it is about 40mins long.
But how many hours will it take for you to reach that level?

Whenever a kid said "I finished Super Mario Land" the other kids would say "Who gives a shit?" because it's easy as fuck.
Now, whenever someone said "I finished Kid Chameleon" people would flip their shit and the kid would be awesome, until they realized he was lying.

Games were hard because you had to earn the right to see the ending.
