Johto is a ton more rural than Kanto. Outside Goldenrod, a city with direct ties to Kanto due to the Magnet Train promoting back-and-forth visitation, there are no urban centers. The majority of cities are small and cozy, and the terrain around them is often hazardous (Azalea is between a forest and cave, Mahogany is between two caves, Cianwood is surrounded by water and mountain, Blackthorn is at the top of a mountain with no easy way to get there other than flight). You just have to look at Route 29 and how windy and relatively long it is, and compare it to the first routes of any other region, to get an idea of how it differs from most regions.

Sinnoh is probably the closest to Johto in that regard because of how mountainous it is, but it still has more large cities than Johto between Jubilife, Hearthome, and to a lesser extent Veilstone.

To be fair, as much as I like generation 2, the best part of those games was revisiting Kanto, Johto is kinda forgettable, it doesn't even have its own Elite 4.

I have no proof of this but I don't think this was planned until the last second when they added Kanto, because of how the routes leading up to the League are completely different in layout compared to Gen 1.
