Once you max it's friendship, you have to level it up during the day. Also, affection is not the same thing as friendship so refresh will not help you.

The easiest way is to just hatch an Eevee. You get a bunch of easy happiness just leveling it up from lv 1.

The lil hearts you see are affection. Not happiness. Those are two different things
