You can make shit as big or small as you want in a game. I would argue that Kanto in the game is no bigger than a few towns. And there is plenty of ruins, caves and forests to base dungeons out of.

The biggest problem, I think, is that the fauna here is kinda shitty. So, you'd have to tap a lot more into the culture. I mean, unless you want the pokedex to be mostly a rehash of birds, lizards and frogs from the previous regions.
