Rutile Ruby/Star Sapphire, it doesn't change the story or add new pokemon/edit pokemon stats but every trainer has been given new teams and a lot of other things are tweaked.

Trainer pokemon all have perfect IVs and usually have a item and competitive moveset/ability. A lot get full teams, and megas can be seen outside of the E4/VIP NPC battles. each gym leader has a full team, most also get megas.

The levels of NPC pokemon have all been edited so that by the time you are at the ~8th gym you are battling level 100s (and should be around there yourself with the use of EXP share).

From the beginning the "foreign" pokemon are available in routes, usually they are only available after defeating the box legendary. Lots of wild Pokemon have a chance to hold items like fossils, the stone required to evolve them or their megastone.

EV training is also made easier by making the first few routes have a 100% chance of a horde that will give you specific EVs as well as the stores selling honey before the first badge and after the second.

Stores also carry a lot of new items (Slateport has competitive items in the market) as well as adjusted prices (pokeballs are cheap).

Eggs hatch really quick and the hatching O-Powers last a lot longer and upgrade quick. Trade evolutions also have more convenient evolution methods.

Forgot to mention, there are also 3 builds.

Legit build means that all routes have the same wild encounters as the base game (levels and pokemon availability) so that you can use the pokemon online without having to make their encounter data legit.

649 build has a lot of Pokemon added to the routes that would otherwise not be available, you'll have to edit the encounter data of pokemon you want to use outside the game.

Leveled build is the 649 build except the levels of wild pokemon are adjusted to fit the level curve, as wild pokemon eventually become severely underleveled in the legit build.

Also there's an objectively bad but interesting hack I played a while back called Ruby destiny reign of legends.

Pretty bad fanfic-tier story but the setting is kind of cool and it feels pretty different from any other pokemon game.

If you can find them, Pokemon Firered: Tentaquil Edition and Pokemon Emerald: Tentaquil Edition. Literally just Firered and Emerald respectively but with Tentaquil as your starter.
