When you transfer/trade old pokemon to new games, you're actually deleting that pokemon from existence and creating an exact copy. The original one is gone forever.

When you have a Pokemon Lv. 51+ the game creates a copy of that Pokemon stats, ability, held item, and moves according to Lv. 50, that's why after each battle the consumable held item returns.

I don't know why certain battle items aren't replenishable after usage. It makes sense for berries, but not so much for items like Focus Sashes (the Air Balloon makes sense as well, because it's assumed it's popped by a non-ground type move, but still both the AB and other one time usage items are too damn expensive.
