You should at least be able to use Toxic on a poisoned pokémon to upgrade it. Sometimes you can ruin the game because one "lucky" Sludge Bomb.

Not at all, anything with a poisoning move is going to know Toxic to begin with.
What they should do is drop Toxic to 55% accuracy, except Poison types who get 100% accuracy. Either that or radically drop the distribution.

Make extra poison (not toxic) instances do 12.5% of the mon's total hp when they would be applied, factoring resists and weakness desu.
Makes burst poison damage more of a threat and diferentiates it from tox.

Not every poison type could just whip up some poison to poison anything it gets hit or is touched by. It needs to be the same thing
Poison touch and poison body need to be the same ability. That now also causes deadly poison.
Poison lowers all stats by 2 degrees on top of applying poison.
