Even if the Wobbuffet use Struggle, the damage will be healed quickly by Leftovers.

It does now but in Gen III Struggle did 50% of the damage dealt IIRC and Wobbuffet has tonnes of HP and no attack.

Struggle now takes 25% of your health. Before gen iv it only did 50% recoil (50% of the damage it caused).

The reason Trainers didn't use Ditto for so long was because if the only Pokemon you had left was a Ditto the two would enter into an infinite loop of endless transforming, where neither would ever run out of PP to use Transform.
They later changed Transform to not copy the PP of Transform if the Pokemon they Transformed into knew Transform.
