Use a gen 3 save editor, then copy/paste the information you see there in Pkhex. I don't know if you need to bother with the trash bytes at the end of the OT/Pokemon nickname, and you should set the location to whatever it uses for mons sent up through gen 5, but other than those its all straight forward.

Transfer the save to your computer and use an emulator to emulate a 4th gen game so you can migrate it.
After that, I used a flash card and sent it over to a 5th gen game.
There's probably a faster way but that's what I did a long time ago.
