I once had a group that took 45 minutes to get the first boss down to 40%. I was on my healer, before you say anything. At one point, the boss approached the gate and the tank kept tanking him there. Every DPS backed off of the boss to give the tank, who could barely hold aggro, all the freedom in the world to reposition. He didn't. The boss moved through the gate and reset. This is why you should always inspect and votekick anyone in IMS that has even just one wrong crystal. I learned to do this from using DPS meters - players with even just the slightest stupid crystel setup tend to have no fucking clue what they are doing. Not him but things were difficult long before that. Almost all of it is BHS's fault for failing to keep people interested in their game. I don't care how good your guild is. If the game can't hold players then your guild will never keep anyone longer than a month unless they have strong emotional ties to the place. Doesn't matter if you have multiple active officers working hard setting up events and running dungeons or just a few autists who still care, if the core game isn't engaging, then people will play something else. That is the case with TERA.
So like any TERA guild, Dump has problems keeping people. That issue is compounded by the officers' stubborn adherence to a 4chan-only rule on recruitment, despite being on the niche roleplaying server that hardly anyone from /vg/ joins under. Unlike public guilds or 4chan guilds on MT that get new people every day, Dump gets a new person every week at best. And like any guild, whether they stick around after reaching the level cap is a toss up. Activity problems are further worsened by people gquitting when they feel the guild isn't alive enough. Many times, though certainly not every time, these people have not put much effort into actually forming parties or teaching newer members themselves, preferring instead to join skilled guilds that others have built already. Which, while irksome, is completely understandable, since building up a guild is hard work and people quit all the time despite even your best efforts. Sometimes you just get tired of every run every day being a teaching run for people who end up quitting a week after they get geared. I'm getting tired, myself.
Of course, this doesn't account for people leaving due to personal issues, disagreements with some members, people who tried to start content but failed due to lack of interest, etc. That's its own thing. I feel like the issues discussed are the big reasons 4chan guilds in general don't last very long unless they assimilate.

You'll need a lot of free time, patience, and passion for whichever game you're playing. If you've got the time and the patience to constantly be showing people new content regardless of their skill level, and the passion for the game to be able to keep doing it while improving yourself so that you can potentially guide multiple rookies through hard content, then maybe you'll be okay. When any of those factors disappear, you won't be very effective at all.
It helps a lot if you have other people in the guild who share this mindset and want to help make the guild decent. If you start your guild with two other friends for example, then you have extra people who can help with things when you're not online. But when it gets to a point where the majority of the guild's founders (or officers) don't even play the game anymore, then the people left who still give a shit will burn out sooner or later. So, ideally, you would need a few skilled people who also still enjoy the game and have the patience and free time to help others.
Lastly, you need to be cool with the knowledge that no matter what you do, people are gonna just fucking leave regardless, for god knows what reasons. Otherwise you'll burn out hella quick when the dude you just spent a week teaching and gearing quits for his friend's guild, or for another game entirely. This game has a ton of slow moments and a lot of downtime, so even if you're handing out free cookies or whatever, people will still just quit due to disinterest. It's hard as fuck but you can't let yourself get upset over it or you'll turn bitter, and then you won't be effective at managing things and creating a healthy and fun environment.

It helps as well to have a solid vetting process for new recruits. Don't make a 4chan guild or you'll run into the problem mentioned earlier, with the pressure to recruit outside of 4chan coming at odds with the expectation that your guild be 4chan-only. From its outset, any guild that hopes to thrive and do high end content without relying on others won't be a 4chan guild because you'll need new members coming in constantly to balance out the never ending bleed of players. Make your guild open recruitment, but try to talk to people before inviting them in. Get a sense for what they enjoy in the game, and if that is in sync with where you want your guild to go. Try and feel out if they seem like the type to join, get geared and leave or if their last MMO was WoW and they were the final person in their guild. I'm not saying only invite people who seem skilled, but I am saying that each person you gear and teach is a time investment that could have been spent elsewhere, so use your time wisely.
Foster some sense of loyalty among your members, preferably by leading through example. If you're trying to do Kalivan's Dreadnaught and you need a healer and there are two healers online in your guild, try asking them before you grab someone from LFG who's not even in your guild to begin with. This way someone from your guild gets practice, and maybe you can even get a fucking guild quest going because of it. Like I said earlier, doing this constantly is exhausting and it's always tempting as fuck (for me at least, never said I was perfect on this shit) to want to IMS or LFG real quick instead of asking people one on one -- and you can totally do that if you want -- but your guild's gonna degrade, you'll lose members faster because they'll get the sense that they aren't really necessary, especially in smaller guilds, and that's when people stop signing in or gquit.

Dump may have started as a casual guild, but you can't deny that it became a content guild as early back as the lucid patch, when their players were farming TSHM for lucid mats. Hell, I participated in RMHM score runs with that guild last patch, so your broad claim about dump not being a place for people who care about their performance has been false for a while. Dump's had people of varying skill levels for over a year now, with a lot of mid-ranged people, a few people who got VM and tried to tryhard as best they could, and some people on the lower end who wouldn't do anything beyond channelworks. Why do people ask questions like this?
If you want to play on TR, go play on TR. If it doesn't seem active there, make it active yourself. Active servers don't become active overnight. Make your own LFGs and fill them with people who want to do content but feel like nobody else wants to. You do that long enough and you make friends, and then suddenly you wake up and have a static.

The boss performs his double debuff mechanic 3 times. The first time, the entire group dies and must get up with neo resses. The second time, the priest pulls it off flawlessly. And the third time, everyone but the priest and two other party members die in the attack.
While I would not call this optimal by any stretch, you're right that it is indeed possible.

It's definitely possible, I did the plague alone once because of an ims healer but the numbers that hit you are so random, sometimes it kills you sometimes it doesn't (always leaves you at <10% health), and why rely on rng?
Also it's better to have double healer in case someone dies in the fire, one healer can go on rez duty while the other takes care of the rest
