Because it streams a lot of textures. It's 2017, it's time to move on and buy an SSD. They are common and necessary today. HDDs serve as a common denominator and are really holding the game industry back, the performance is many times worse than SSD. (the same with sRGB and many other things)

Possibly failing VRAM or a bad display cable. Run Unigine Heaven once and watch for any textures that don't display properly. Monitor with GPU-Z or something.
