Scat sometimes just happens in a scene while NTR takes some build up. So people get a warning about scat as it would be surprise scat without it most times and you don't need one for NTR seeing as surprise NTR is pretty rare due to it not really being good NTR if it's a surprise. From what I understand it's kind of shit in the NTR department.
The point is though is that through the story and context of what is going on you can tell when a girl is in a relationship and is having sex with people outside that relationship (either her being raped or tricked or doing it because she wants to), where as scat just kinda happens in scenes sometimes with no build up to it or anything which is why it gets the warnings for it, you'd have little to no way of knowing that scat was ahead without it (not all anal sex has scat and when it does people like to know before hand).
