A bunch of gem quality is merged, refers to the chart for details
Unless you are using some shitty crafted gems, you are not getting affected.

One major change is that citrine's life drain will work on an extra cc (daze), sapphire getting 2 crits, and peridot works on block

I believe the effect is across-the-board, and the gem quality only affect the strength of the effect

The % mods are bonus AP ratio, not direct damge multipliers, ex. A skill with 10 AP mod before raven and +300% on raven would be 13 AP Mod after, not 30.
