Swallowed penny from a first floor pot isn't that rare, from there you just need IV bag and restock and you've got a break going, neither of which is that rare either. He's not that hard to break and frankly you can win without breaking.

Greedier mode isn't that hard for keeper due to all the health drops. As for the Lost, pray for 9 lives, steam sale, or massive damage. I did Greedier for both without breaks, but admittedly my Keeper run was one of the luckiest I've ever been short of that time I triggered a glitch and one shotted Greed's second form. We're talking triple shot sacred heart god head Greed's first form melted so fast he didn't even start his second attack.

Ultra Hard, hold R for library. The library pool is fairly small and Book of Revelations/Satanic Bible wins you the run. From there it's literally just not picking up Bucket of Lard/not getting total crap damage until Mega Satan.

Basically no bullet-related upgrades whatsoever, you just get new guns which mostly suck and stat buffs. This kills the replayability because the vast majority of the guns aren't mechanically any different to begin with. Every run essentially feels like a tears-only run in Isaac.

At it's core, replayability is the biggest factor, a factor that's mostly provided by randomization and procedural generation of content.
Even in real roguelikes, there's only so many enemies you can have, so many layouts a dungeon or cave can have, and it falls to other factors to create a different experience every time you play.

The experience in Isaac is given by the items you find or decide to work for. You could have two runs with identical level layouts and characters that play and feel wildly different by changing two or three important item pickups.

I love Nuclear Throne, more than Isaac, but after only 200 hours I can comfortably say I've had just about every possible experience possible from it. I still play it, sure, but it's using the same methods and strategies I've been using forever.

In Isaac you're constantly forced to adapt to new playstyles and develop new strategies based on the tools given to you, which is why people can play for 500+ hours and still not be bored with it.

I'm not saying it's a great game, but it's perfect at fitting the genre.

Essentially Isaac is fucking bonkers. He has an overactive imagination, and he lives under the roof of a southern catholic cunt who is strict as shit in the name of religion, and the concept is scary as shit to Isaac because she would do anything in the name of religion, even if it meant killing him.
The game is a combination of hallucinations, split personalities and what-if fantasies ("I hate all this stupid god stuff... What if I found the Devil and made him my friend? I bet he could make me a demon, too! That would show her..."-Making 3 deals with the devil in one run unlocks Azazel, demon Isaac)

The various implications and endings to these what-ifs are that maybe it's really the Devil making her do all these mean things(Satan boss), Or maybe there's something wrong in her head like a tumor that's making her crazy(Hush), or maybe she's just fucking insane and doesn't know what she's doing(Delirium). There's also the implications of what happens to Isaac, like he suffocated from hiding from her (???), or ran away and died (The Lost), which I believe is the true end to Isaac, which is why The Lost was supposed to be this huge, collaborative community effort.

the whole game is a representation of isaac's thoughts and feelings about life
many of the bosses and enemies represent aspects or elements of isaac that piece together into the large sum of his internal struggle against all of the shit that makes him sad, which he eventually loses when he kills himself
the cathedral and chest bosses are isaac's body, one of which literally being his dead body inside of where he died.
the dark room boss is isaac's soul, or at least his own interpretation of it, as he's been guilted into believing he's some sort of evil satan baby.
Delirium is Isaac's mind, not a figurative imagination of it but literally the last reasoning that Isaac has left as he's dying, struggling to call out to him and make him stop.
The blue womb and the void are both Isaac's imagination failing slowly as he dies, which is probably pretty stressful since he's not only killing himself but doing so in a very unpleasant manner
The blue womb is just a recolored womb that has no actual floor, and the boss is actually just a fucking blob of blue shit(I love his design, but still). Isaac at this point is losing the ability to think up new shit to go along. It also appears after he goes quickly in the game, which could suggest that every single run is actually canon and that the runs get shorter as the end of his life approaches
The Void is the next step after the blue womb both in progression and symbolism, at this stage Isaac has lost all ability to think coherently and just throws together ideas with no real rhyme or reason- basement and womb rooms, isaac and satan bosses, and a boss who looks like dying memories of every other boss meshed together. the floor's static and shit are basically his last dying hopes for survival jutting into his mind and screaming to him, which is why they look so unnatural and jarring(and I really liked one-frame delirium because he fit that perfectly).

All item pools are reset upon starting your next run and you're actually really likely to get a few of the same items from run to run so you don't need to worry about what you pick up and instead just go for broke.

Blank Card+Black Rune's even more effective than Void because 4 room charge+you can eat active items that you frankly don't need with swole stats. Since you run the risk of losing your active item at the start of each loop, I'd straight up Black Rune Void in that situation rather than voiding Blank Card.

In fact, I almost did on my run but I didn't want to spend the hp to get Void at that moment.

I know that feel, bro. When I was going for RERUN I got Epic Fetus from a Dark Room curse of the blind after accidentally bumping into the chest after deciding fuck curse of the blind.

As it turns out that shit with the bookworm transformation straight up doesn't work.

This was literally the Dark Room right before the final lap as the lost. Fortunately I saved a double charged shovel and an Ehwaz rune for a rainy day and had ridiculous screen clearing ability even into the next run so between Guppy's Collar and Judas's shadow I managed to see the end of the next run.
