The character Kokichi Ouma is certifiably a madman and evil. Whomever he was before that is more ambiguous. We don't know his intentions for signing up for the game, but they could have been for despair/suffering.
It's a pretty despairing move to sign up for a public killing game as a shsl despair when you've done nothing but help people before.
