Almost no autism at all. There are people who dig minecart tunnels the span of a whole large world. But I'd recommend using teleporters instead of minecarts. Or maybe making a minecart up in the sky.

Els are the best.
You can start building them day one, they can be used for fighting bosses, drive-byes during invasions, can be grappled to for a quick escape, even farming if you build them at just the right elevation above the ground.
Light them up with living fire blocks in hardmode.

Came back to this game after 2 years and I wanted to try expert modo. Turns out it's brutal af and I'm getting my shit kicked. I've also heard all the bosses are near impossible or some gay shit like that

At what point should i fight cthulu eye? I have full molten armor ATM but still feel unprepared

Molten armor is pretty ok. Your best bet is to get a really good ranged weapon. And build an arena with enough room to kite the bosses. All of the expert mode bosses have increased health and damage as well as new mechanics.
