(I specifically say Izayoi is the fourth victim because we already knew Seiko was the third victim. As of this episode, Miaya was dead from the very beginning, so she technically doesn't qualify as the fourth victim)

They've all got something in common.

First Victims: Serve an important plot detail later on
Second Victims: They had a personal, moral resolve
Third Victims: Eccentric but pure of heart
Fourth Victims: Sacrificed themselves for their friends

Maybe...just maybe...Ruruka woke up, killed both Seiko and Yoi. Ruruka probably didn't want to kill Seiko, and definitely didn't want to kill Izayoi. But, if she didn't, something would have happened. So Yoi told her to sacrifice him. At that point, that was when he ate her candy to remember her.

Here is a tip in dealing with white lines.
Make a layer, grab a paint bucket and make it a dark color that will pick up white. And make sure its the back ground while the image is the forground.

Just because we didn't see it on-screen, doesn't mean it didn't happen.
