Carthus was a well designed area in terms of level design, even if it was short.
Anor Londo is just the boss run / 'oh shit i'm here' moment rather than its own area, at the end of the Irithyll area.

The Dungeons were a mostly failed attempt to recapture Tower of Latria, but was relatively well designed and pretty spooky (those eyeless slugmen things in particular)

Profaned Capital and Smouldering Lake are complete shitpiles that actively make the game worse.

The way I see it is that the Dark Souls universe is like a wheel, with the Kiln at the center and the other kingdoms/worlds existing on the 'spokes'. When the fire begins to fade, the worlds all kind of get mashed together ('the transitory lands of the Lords of Cinder converge').

Remember that Gwyn was literally the first person to link the fire, and because he had such a huge dick his soul was enough to maintain the flame almost entirely until Chosen Undead shows up and murders him, at which point he's almost spent.

Anor Londo is not really still around, the only remaining part of it is the main keep, and Irithyll was built around it, clearly many years later. Gwyndolin was still there because he's a God, likely still guarding Gwyn's tomb and handling the Darkmoons, until Pontiff and his asshole clerics from the deep showed up and fed him to Aldritch. It's probably been many many thousands of years since the events of DaS1 (relative to Anor Londo) because the entire area surrounding it has collapsed. My head timeline for Anor Londo is something like:
Flame Fades > Gwyn Leaves > 1000 years pass > Sun has gone out, Gwyndolin maintains illusion > Dark Souls 1 happens (CU canonically doesnt kill Gwyndolin) > Indetermined amount of time passes, the mountain that Anor Londo is on collapses, leaving only the keep and the spinning staircase leading to it > People discover it, construct Irithyll > Dark Souls 3 happens.

Andre is unclear, I would say that he is just an undead that is also woken up by the bell of awakening at the start of DaS3, and he takes his place as the blacksmith in the shrine as he always has. The Blacksmith Hammer mentions that its 'passed down between blacksmiths' though, so who knows.

Overall, I would say that time passes separately for each of the worlds connected to the Kiln but when the fire begins to fade the Kiln's ability to keep them separate is weakened, and so they all get jumbled up together.
