It means you regenerate health based on the damage you deal. If you have 2% leech supporting a gem then you will heal for 2% of the damage that gem does over a short interval. There is also a maximum regeneration rate.

Anyway, it would depend on what weapon you found for what would be decent. Two-handed swords are good but hard to get a good one of. You could go for Immortal King's Maul since that is fairly easy to find, but it's not super amazing in Hell. Then again, Barbarian is pretty crummy in Hell in general; too many physical immunes.

it feels slow and clunky as fuck when you start out but once you get proper support gems and get used to it a bit, it's not an issue and besides, the initial hit is fast, then you can fuck off before the aftershock hits and by that time you're already landing more hits elsewhere

Okay, but also consider you're going to be losing out maybe 20 to 40 life, 160 in assorted resists and 1000 base armour or evasion. For this effect that isn't necessarily going to make the biggest difference for you depending on your build.

It's significantly better if you're an evasion build where it's the equivalent of 7.5% physical damage reduction, right? It's not 7.5%, it's 75% of 30%, which is 22.5% damage reduction. Add in a purity of lightning (which you'll likely use to get your resistances back up anyway) and it's 23.7% pdr.
