Hell,the importer still doesn't accept styles, you have to upload a separate version and then email valve.

Most of TF2's community servers have been literal shit for a long time now.
Top Notch Clan and Hyperion Gaming run decent custom Payload servers, but they're West Coast (I'm East Coast) and they are almost always full during prime time, and also have donator slots, so if you do manage to get on, you're bound to be kicked in an hour or so.
The Furry Pound is okay, but you've gotta deal with that community.

Currently if you've got a syringe gun, switching to it is the worse option over 95% of the time, on anything other than the most braindead pub.
The reason the crossbow is considered by far and away the best medic primary is because it actually offers something worth switching to and using, outside of occasional self defence scenarios where you're without a teammate and aren't already dead.
The remaining 75% of medic's primaries are going to need a pretty significant buff if they're to be used at all.
