Medic never got rescued or went to a hospital, he fell out of the helicopter as it went down and hit his head on some debris.
While he was passed out, he dreamt the whole coma thing up, up to and including rescuing Miller
Then Medic wakes up on what remains of the MSF motherbase, and blindly wanders around it talking to imaginary soldiers and occasionally shooting birds and fish that travel by

After becoming very familiar with the story arc, characters, sub plots etc that are involved in the missions, I recently beat the truth mission after holding off for months and I thought it was a great ending. The character of venom becomes all the more interesting and cool when he embraces his role and decides to fulfill his purpose of becoming a major antagonist and demon in the world's eyes. I thought the ending was satisfying in that it dumped a lot of revelations on the player at once in an unexpected manner and made you think back on the game story in a different light making you want to replay missions and keep playing in general. My favorite parts were that the ending made me as a player want to stopy trying to be a hero in a sense and start playing lethally and the part where Miller directs all his hate against big boss and wants vengeance. The story was a bit awkward but looking back I think over time it and tpp in general will be seen as a great game
