Everything that happened was real, you were playing as a body double. The body double, or venom snake would build outer heaven.

The real Bigboss went to form Foxhound in America, the idea was to stage a conflict between outer heaven and fox hound. Big Boss would look like he is staging operations against Outer Heaven, but is actually working in unison to cover the tracks of Outer Heaven. He would send soldiers who were either in the know like Gray Fox, or inexperienced soldiers who would they thought would fail like Solid Snake.

Except Solid Snake didn't fail, and took down outer heaven and killed Venom Snake. Big Boss flee's from fox hound and resurrects his operations in Zanzibar Land, which Solid Snake shuts down again, this time nearly killing Big Boss, which sets off the events of the Metal Gear Solid games.

Neat. He didnt lose that eye. Its still there. Matter of fact he didnt get shot in the eye at all. The flash from the barrel burned his eye causing him to lose his vision. Do some research before you post.

The whole point of MGSV is ultimately that it's your story and therefore it's meant to be interpreted in a bunch of ways. The game even outright states this with-
"Facts don't exist, only interpretations."

Basically, the game could literally be DUDE IT WAS ALL A DREAM LMAO and we will never know. This is Kojima's final gift to Metal Gear: Wishy-washy bullshit per usual.
