I really can't think of many that make a big choice out of it besides Bioware games.

The later Persona games I guess, if you're alright with the protagonist having all the charisma of a brick, and the romance never actually being brought up during the main story.

Well, it starts as a political marriage where you choose one of the princesses of the leader races, and you choose from elf(beauty incarnate), dwarf("emphasis on bust"), lizard girl(cold as ice but if you play your cards right she warms up to you and gets actually nice), skelle wife, which I cannot go into detail cause od spoilers.
