What's the issue? Hiding from enemies is easier than gunning them all down, and considering that all Metal Gear protagonists canonically enjoy killing people then you shouldn't really be ecessively penalised for it.

Yeah, I'll pass. At the very least, in keeping with Quiet's backstory of being an assassin, she should have been played by an athletic woman with nice ass and thighs from countless hours of jogging and squats to maintain strength.

The AI is actually really smart though and the more you resist the more they're going to use on you. You still can only take a few hits from regen health even with bandana/armor.
