Good design - sorry basic design taught in 101 level college classes teach designers to account for human errors that have a high likelihood or to at least have testers in R&D to find those errors and fix them.

Do you know why toothpaste caps haven't innovated much? Because objectively better systems run into human error. We've done shit like auto sealing and the testers try to pop the cap off still.

Sliding a black bar with little in the way of denoting orientation aside from a hand strap the wrong way is not only easy to anticipate but easy to design around.

It is a failure of design.
