I would say it's yes to both of these. The gameplay is basically the same, with improvements, but you can buy a LOT of quality of life/gameplay enhancers through the cash shop. The game has no pvp so it's up to you to use/not use it though.

most of the overworld looks like midland, besides the starting beach area. there's also a guts character preset, a casca expy, and in the ps3 and 360 versions you can buy guts and griffith's golden age armour, they lost the license in the pc version though

The combat is fucking amazing, although the difficulty is a bit fucked. It appears to be based around skill, but its all numbers since the game deals with flat numbers instead of percentages. This means that in one minute you can be one shotting everything and in the next you do literally no damage and die in one hit. Its better if you go into it knowing this
the bossfights are the best part, along with the dlc which is just a giant end game dungeon crawl with some of the best bosses in the game
And unfortunately there's no mods to bring the armour back, and all files for it were removed
