Follower ending is an extension of Yes Man. If someone really wanted, they could do and take whatever they want to support the Followers with that ending. No need to rely on NCR.

Karma is basically just used to cold read your ending slides. A Courier with bad karma is most likely going to have killed more people, so ending slides tend to reference an atmosphere of violence and fear.

Whereas a good courier most likely only killed people who objectively deserved to die for the good of the Mojave, and gets more positive ending slides as a result.
