Haven't played it but The Long Dark seems to be all about surviving the elements (of winter at least).

Not quite sure if it's quite what you're looking for, but here's two things that come to mind:

Misery mod for STALKER has a mode where the entire world is radioactive during the daytime, you really have to plan and prepare for every trip.

I haven't played too many myself, but I bet you could turn some of the Bethesda RPG's into this. Fallout 4 has it's Radstorms so you could probably find a mod that really cranks up the radiation damage from that, and that'd be a start.
