One system I actually liked a lot, of all fucking things was Runescape's. The actual elemental aspect never really gets touched upon unfortunately, but if you think about it it's pretty clear that the runes themselves can be divided into distinct groups: traditional elements, 'bodily' elements, elements associated with the three main in-game gods, elements associated with the world, and elements associated with mortality, but the rest are a bit more varied.

I'm making a few simplifications here: the Life Rune was never officially released, though it has been acknowledged, and there was a later god rune that isn't really worth mentioning so I'm ignoring it.) While the system itself had many flaws, the lore behind it was really neat.

Magic attacks ought to have physical modifiers too. An Ice Spear attack should be both Water and Pierce. The two types should not be mutually exclusive.

Water, air and earth are all matter combined with kinetic force, so yes, that would be physical. But fire is just heat/energy with no mass behind it. It does not hurt you by hitting you hard, but by denaturing the proteins that make up the tissues of your body (burning you). So in a sense it would be energy-based damage, not physical based.
