The moment a game is rated AO, it's pulled off store shelves everywhere, and retailers will refuse to stock it. Just ask GTA San Andreas after Hot Coffee, or Manhunt 2.
The fact is that if your game is rated AO, it's practically blacklisted.

1. Most people still see vidya as a hobby for children. Sex games fill this weird area where things that were typically for children are sexualized that makes most people uncomfortable. Hentai has the same problem.
2. The uncanny valley is a real thing. Video Game characters don't look exactly like real people, and in some cases, it's offputting.
3. America has a problem with sexuality in general, specifically female sexuality. The Right has the fundamentalist Christians who think that porn is the devil and women should be subservient and non-sexualized. The Left has radical feminists, who consider porn as a form of violence against women, and that being attractive to men is a form of oppression that women must actively avoid in order to fight patriarchy. These groups aren't that large, but are very loud and influential in politics and media. It's the same reason prostitution hasn't been legalized, despite showing it's effects on lowering crime, preventing STDs, and creating a small economic boost.
4. Sex doesn't make for a good gameplay mechanic. Most normal people would rather just watch porn.
