In these cases multiple people report the names, not just one, as confirmed when the email says "multiple times". This is a case of 2+ players reporting the name, and the mod agreeing that it's breaking the rules. If the player wasn't a faggot they wouldn't have been reported in the first place, though.

Back when I worked there (like 2007-2008 ish) it was left up to the discretion of whoever was handling the incident.
I used to pick the fruitiest and most effeminate, yet nonoffensive names if they had a male character, and completely mundane, boring, or male names if they had a female character. And they were stuck with whatever you picked unless they paid to change it.

Was really funny to check up on them after their associated suspension was up and see how other people reacted to their name, you could feel the shame radiating out of the screen. Sadly I don't think GMs can get away with being as cheeky as they used to, people are even thinner skinned than they were back then and naming a manly orc warrior to 'Twinkletoes' would probably constitute harassment
