Bethesda doesn't want to make RPGs anymore and wants to stick to making open world games, more ala GTA. However the IPs they own have reputations for being RPGs, especially Fallout, so they can't strip them out and alienate too many of the fans. At the same time they don't want to take a risk and start their own, new IP that is strictly and open world game.

points out, it's not that they're totally incompetent. It's that they don't care. Since Skyrim's explosion in popularity they realize (and you can probably trace this back to Morrowind) that actual depth and complexity is hard and it's more profitable to cater to the lowest possible denominator, make you game as shallow as possible and hype it to hell and back.

FO4 only really had two marginal things it improved over NV, which was shooting and a very slightly less buggy product.

Fallout 4 wasn't a disaster, however bethesda are not that ahead of most of other open world developers now as they once were. FO4 didn't have enough improvements to really justify it as a numbered sequel.

FO4 does get some props for being the least buggy bethesda game. However it also needs to get deducted points on how badly they gimped the modding community.
