What are some good examples of games that do it well?

Morality systems that is visible to the player simply don't work, because the player will always try and manipulate it. Although I can't say a hidden one would be that much better either, because it still puts a binary restriction on the player's choices.
Why confine moral dilemmas to a system at all? The best option is to just write quests in the way that would offer the player the most choices.
Simple action-consequence is a much more elegant and powerful system, than any kind of binary good-evil or lawful-chaotic system a video game can implement.
