Well, I suppose Counter Strike, Team Fortress, Dota and DayZ are the most historically significant. Every single one formed an entire genre that then became an industry-possessing madness for a while.
Red Orchestra and E.Y.E also started as mods before they went commerical. And Dear Esther, even if you might not like it (I don't) also proved out to be a herald of what became an entire new and very successful genre.

My personal all-time favorites however would be:
Complex for HW2
Dark Mod for Doom 3 (still the ONLY good game ever made in Doom 3's engine)
Nameless Mod for DX
Black Mesa for HL2
Angel+Bob's mods for Factorio
