You can not name anything from any media in the past 3 years that has never been done before. The entertainment industry is so gigantic with so much content being pumped out that there is nothing that has never been done before to a certain extent. The line for "never been done before" is rather arbitrary as well. It is impossible to make a game that has entirely original mechanics, looks, story, and even backend/frontend framework.

How many variables must be unique?

Exanima. A 3d dungeon crawling roguelike with permadeath, physics based animations, momentum calculated damage, many layered armor and penetration system, physics manipulation puzzle solving, randomized loot and arena fighting game. I could go on.

Can you name xx amounts of 3d dungeon crawlers with permadeath? Most likely but I'd still argue this separates itself enough from most to feel unique. With all the cons and pros that follow.
