Getting a weird glitch in Fallout 4, dunno where to go.

My companions wont go to the settlements I tell them to, nor do they return back to their starting place on the map if I dismiss them. They literally just walk off the fucking map and are gone until I recall them, which they then go and walk off into the sunset again unless I ask them to follow me.

What gives?
