Factually untrue, if you have half a brain and play pacifist the first time you'll get the neutral ending and be able to start over without much setback and just do what's required for the pacifist ending, then the genocide run is a separate thing entirely. This is what literally every other classic rpg /action adventure does though. Unless you have many savestates to chose from you'll have to replay Silent Hill to get the UFO ending. If it doesn't work out, remember that you don't need a grand story, a vast world or many mechanics in your game. You can make profitable smaller-scale, high-concept games. Look up The Cat Machine on Steam. It was mostly made by one guy and he said it pays his living expenses.

Some people don't watch trailers. I don't because I hate spoilers. Even so, the game made it very evident that it wanted me to play nice, so I figured that doing so would reward me and I did it. It was about as clear and up-front about the whole thing as Bioshock was. it's more common in games (in a situation like this) for the storyline to pull a fast one on you and not actually kill them, or for the death to not count or be reversed somehow

Yeah it is an intuitive solution. There's a frog NPC before that point that outright hints you toward being persistent with it. I tried the most obvious solution first, which was being persistent, and when my health got low the boss I was fighting started to intentionally miss me. How is it not intuitive to continue sparing while allowing the boss to miss you? It's really, really obvious.

The only clues you get though is that she will stop getting aggressive as you run out of HP, and her attacks will directly avoid hitting you and deal less damage as a result. So really you have all the time in the world to fuck around with options till something at last shows a new result.
Also a hint frog just before her informs you that sometimes you need to use spare more then once to start seeing results.
