I'd put it the other way around. If you add in the DLC it's about 1/5 looking for Ciri 4/5 side quests

Question: so I'm in Novigrad and I've been playing really stupidly. Only been leveling combat, not much money, only have my base items. Should I start over, or is it possible to push ahead no matter how shitty your skills and inventory are?

If you get good enough at dodging and using the shield spell you can go through the entire game like that.
I rarely leveled spells aside from the conversation one and got through it fine.
If you do treasure hunts you can get some okay equipment that sells pretty well for the most part.
If you are skipping side quests I'd suggest dabbling in them somewhat as they're pretty well done for the most part aside from the "follow the red footprints" ones.

The game is set up so that main missions always give a ton of experience. Even when you think talking to Yen is worthless, you see the experience gained, and find inspiration to continue.

I will admit, on my second playthrough, I ignored damn near every sidequest and low level item. Quests are there merely for world building and frustration.

No the rest is spend looking for red tracks on the ground

There sure is. After you find the anime magical princess, she joins up with the anime swordsmen to fight the evil transdimensional army trying to invade Earth!
