Antibirth has more enemies and bosses, but Afterbirth has more items. They both add the same amount of characters, but Bethany > Lilith. J&E and the Keeper can both suck my dick.

I personally prefer Antibirth for the new Zodiac items, Red Key, and its alt-path. The Corpse bosses in particular are all fantastic, and enemy design puts AB and AB+ to shame.
